Privacy & Consent Notice

This notice describes the SSDI Advocate website at help.ssdiadvocate.us, operated by Go Referral Agent LLC. It explains the information used in a review request and the consent documentation that accompanies it.

Information you choose to provide

The questionnaire asks about your work history, disability benefit application status, representation, and how a health condition affects work. If you choose to request contact, it asks for your name, email, phone number, birth date, and address. Some of this is sensitive health or personal information. Do not submit Social Security numbers, medical records, bank details, or other information the form does not request.

How we use and share a request

We use a submitted request to connect you with participating providers, document your instructions and consent, operate the form, and investigate errors or misuse. When you submit, the information is sent to our lead management system, LeadProsper, and may be shared with participating providers identified in the contact disclosure you agreed to. We may receive compensation for making these connections. Submitting does not guarantee that a provider will accept your case or contact you.

Consent documentation and website activity

The form uses Jornaya, TrustedForm, and LeadProof to document the disclosure presented and your interaction with it. Depending on the service, documentation may include page content, linked policies, form interactions or entries, a consent choice, timestamps, device and browser information, and IP address. The record is associated with your request using consent identifiers or certificate references.

ClickGo supplies campaign attribution. The website carries campaign click and publisher identifiers with a submitted request. Contact details, birth dates, and health descriptions are not placed into our ClickGo conversion URL. Our hosting provider, Bluehost, and service providers used to operate these systems may process information for those services.

Your choices

You can leave the questionnaire without submitting a request. Marketing contact requires the affirmative contact choice shown in the form. Consent is not a condition of buying goods or services. To stop texts, reply STOP to the sender; reply HELP for help. For calls or other communications, tell the contacting provider that you are withdrawing consent. Contact each provider that is communicating with you.

You can also use our Information Preferences page or email support@goreferralagent.com to request assistance with your information or withdraw a contact request. Depending on applicable law, you may have rights to access, correct, delete, or restrict uses of your information. We may need to verify a request, and legal recordkeeping obligations may limit deletion of consent evidence.

Retention and protection

Review requests and consent evidence are held in the systems used to process and document them. Retention depends on the purpose of the record, service arrangements, and applicable legal obligations. We use HTTPS for form submission and keep service credentials out of public website files. No website or transmission can be guaranteed completely secure.

Other providers and changes

Participating providers operate independently and may have their own privacy notices and communication practices. This notice does not govern their independent services. Updates to this website’s notice will be posted here. Read the notice and form disclosure that apply when you submit.
